    What This Person Will Do & The Impact They’ll Have

    This role oversees four teams, with preference given to candidates who live in East Tennessee and who have worked directly within the CHOICES program. In addition, experience providing care coordination to persons receiving long-term care and/or home and community based services is essential for this role.

    Their mission will include:

    • Leading, supporting, and developing four teams to succeed both at the enterprise level and within each team’s unique culture

    • Strengthening morale, engagement, and accountability

    • Ensuring operational excellence through consistent processes, reporting, and communication

    • Serving as a connector between teams and other departments to ensure alignment and collaboration

    • Driving performance, quality, and continuous improvement across all teams

    Job Responsibilities

    • Evaluate and enhance the efficiency, effectiveness, and quality of work flow, and lead performance improvement efforts to identify opportunities for improvement and to implement best practice solutions.
    • Manage supervisory level personnel and care coordination staff, as well as hiring and ongoing training of the care coordination team. Interact, facilitate, and establish partnerships and collaborations with internal and external stakeholder.
    • Develop and maintain reporting mechanisms to allow for tracking, trending, and monitoring of initiatives and outcomes. Facilitate gathering, analyze, and present data to comply with reporting requirements.
    • Ensure compliance with federal, state, and agency policies, procedures, and standards through documentation review, clinical outcomes analysis, and staff education.
    • Valid driver's license required.
    • 90% day travel required.
    • Various immunizations &/or associated medical tests may be required for this position.

    Job Qualifications

    Education/Licensure

    • Registered nurse with an active license in the state of Tennessee or hold a license in the state of their residence if the state is participating in the Nurse Licensure Compact Act; OR, Masters in Social Work with an active unrestricted license.

    Experience

    • 5 years - Supervisory experience required
    • 3 years - Experience providing care coordination to persons receiving long-term care and/or home and community based services required

    Skills\Certifications

    • Analytical and decision making abilities.
    • Proficient in Microsoft Office (Outlook, Word, Excel and PowerPoint)
    • Excellent oral and written communication skills
    • Good interpersonal skills and teamwork ethic
    • Be self-motivated and capable of working independently
    • Be organized, reliable, and able to manage multiple tasks with exceptional work ethic.
    • Ability to lead, train, motivate, delegate, follow-up, resolve and overcome obstacles to meet aggressive time tables.
    Employees who are required to operate either a BCBST-owned vehicle or a personal or rental vehicle for company business on a routine basis* will be automatically enrolled into the BCBST Driver Safety Program.   The employee will also be required to adhere to the guidelines set forth through the program.  This includes, maintaining a valid driver’s license, auto insurance compliance with minimum liability requirements; as defined in the “Use of Non BCBST-Owned Vehicle” Policy (for employees driving personal or rental vehicles only); and maintaining an acceptable motor vehicle record (MVR). *The definition for "routine basis" is defined as daily, weekly or at regularly schedule times.
